Every child is smart and if they are taught properly they can accomplish anything they dream of.... In a first session, I would learn about the student and introduce myself so the student is comfortable with me. I would then go over the subjects of concern and gather information on what the student's strengths and weaknesses are. I would also ask the student what resources they use, such as textbooks or calculators, to ensure they have everything they need. After the session, I would write up a lesson plan based on my assessment of the child's needs.
