"The student and I worked on problems from the Chapter 2 homework that she found confusing/difficult. We tried to get through as many problems as possible but did not get through them all. She will follow up with me via email/phone if she has any other problems that she would like to discuss."
"Today we worked on editing the student's personal essay assignment. She has a very good grasp on what she needed to improve, and we talked about different means of achieving her writing goals. By the end of the lesson she knew what to do for her conclusion and was equipped to edit the remainder of the essay."
