"In this session, we reviewed two of her previous quizzes. We went over each item that she answered incorrectly. I explained why, and we discussed the correct answer. I offered ways to help her with similar types of problems in the future."
"Review for the chapter 2 test. He typically understands the steps he needs to take to complete a problem. Errors typically occur in the execution of those steps. We went over strategies for solving "percent of change" word problems. Moving forward, he should show all of his work and double check that work before submitting assignments. I provided him with extra take-home practice on subtracting rational numbers. I will reassess this at the beginning of next session."

Completed homework problems for 2-3. She solved homework problems with minimal errors. Reviewed simplifying strategies for expressions involving 3+ terms."
